New Patient Registration
If you live within the Practice area, shown on this map, you may register with the practice for General Medical Services. To do this you should ask at Reception for a Registration Form which you will need to complete and sign. For children, or other members of the family who cannot attend the surgery but have given their permission, an adult member of the family may sign on their behalf.
When registering you will be asked for proof of ID, including your current address, and to state a preferred doctor although you will be a patient of the practice and not that specific doctor. However, when attending for treatment you may see any of the partners.
The Practice does not discriminate on the grounds of the applicant's race, gender, social class, age, religion, sexual orientation, appearance, disability or medical condition. In the case of applications made to join the Practice list grounds for refusal include that the applicant does not live in the contractor's practice area.
Registration Form
You may print of the 'registration form' from this website - but this does not mean you are accepted by the practice you will still need to bring the form into reception for processing.
